How We Help You
We’ve teamed up with the American Heart Association to bring validated blood pressure monitors right to your home. We’ll train you step-by-step on cuff placement, posture, and logging your readings. Whenever your numbers look off, we’ll guide you on exactly when to reach out to your health care provider.

You can join us if you’re:
- Pregnant or within six months postpartum
- Uninsured, on Medicaid, or without a regular source of health care
- Living in Lafayette or a nearby Missouri county

Four Simple Steps

Orientation
We’ll meet you in person or online for a 15-minute demo: how to position your arm, rest before a reading, and log results.

Take It Home
You borrow a clinically validated monitor for up to 12 weeks — no cost to you. Just complete our borrower’s agreement.

Track & Share
Use our easy BP log (we’ll provide the PDF or paper copy) to record your readings and let us know if anything concerns you.

Return or Extend
Bring the monitor back when your loan period ends. If you need more time, just ask — we’ll work with you.

FAQs
Can I participate if I live outside Lexington or Lafayette County?
Currently, the program is designed for residents in Lexington, Lafayette County, and surrounding rural areas. Please contact us to check eligibility if you live elsewhere.
How long can I keep the monitor?
Up to 12 weeks. Need more time? Let us know.
Is this a substitute for medical care?
No. Anytime your readings seem high or you feel unwell, you should contact your provider right away.
What if I lose or damage the monitor?
Please tell us as soon as possible. You may be responsible for replacement.
Will my personal information be shared?
No – all data we report to our partners is fully de-identified. We only use your readings in aggregate.
Is there a cost to participate?
No. Participation, including receiving a cuff and education, is completely free.
